Viewing Enrollment History

This tab will show you the client's enrollment history. You can use this information to learn more about your client and their participation in other projects. Projects (labeled as Program on this tab) follow the naming convention of AGENCY NAME - Project Name Project Type [Funding Source] SHELTERPOINT (if using ShelterPoint). For more information about a project, you can search for it in ResourcePoint. The Project Start Date will show you when they were enrolled in the project, and the Exit Date is when they left. If there is no Exit Date, the client is still enrolled in that project.

Creating an Entry

Click Add Entry/Exit to create a new entry assessment.

Make sure you select all clients who should be on this entry. Then select your specific project as the provider and the assessment type (hint: if you do not know your type, select any type and then you can update it on the next screen). Enter the Project Start Date and click Save & Continue.


The next screen should show you assessment questions. If you see a message that says "No Assessment has been specified for this Provider" instead of questions, do not close the entry! This means that you selected the incorrect type for the provider. Scroll to the top of the entry, make the necessary corrections, and click Update. If you do not know your assessment type, you may have to try a few different types until an assessment loads.


On the left side of the screen, you will see that one client's name is highlighted in blue. All of the questions that you fill out will be answered for this client. Then you will scroll back to the top of the screen, select the next client and answer questions for them, repeating this process until all clients on the entry have completed assessments. You can answer the questions that apply to all clients at once by clicking the Add Household Data button in the top right of the screen.


The colored bar next to each question indicates how old the response is. Colors range from bright green (very new) to red (old). Regardless of the color of the bar, you must always ask the client all questions in your entry assessment. These responses have rolled forward from previous assessments and it is your responsibility to confirm whether they are still accurate, or to update the response if it has changed.


See the Entry Assessment Tips guide for more details on how to answer specific questions. Remember to click on each client on your entry and confirm that all questions are complete before you click Save & Exit at the bottom of the assessment.


Creating an Exit


The guidelines for when a client should exit your project vary based on project type. Some projects auto-exit all their clients at the end of the fiscal year, so creating an exit is not required. Others may exit clients when they are no longer receiving a service, or if they have not been in contact with the project for a given number of days. When you are ready to create an exit, click the pencil icon next to the empty exit date. 

A pop-up window will appear that asks for some exit information. Make sure you select all household members who are exiting on this date.

  • Exit Date - this varies based on project type. Please make sure the exit occurs after the last service transaction.
  • Reason for Leaving - please be as accurate as possible and do not select any of the options that end with (HOPWA Only). If you select Other, please give more information in the field underneath
  • Destination - see the Living Situations and Exit Destination Crosswalk if you are unsure what option to select. This can be entered based on client self report, or HMIS activity you see in the client's entry/exits. If you select Other, please give more information in the field underneath

When all questions are complete, click Save & Continue.


Some projects complete an exit assessment. If one appears, please fill out all assessment questions for the clients you are exiting. If no exit assessment appears, you can close the window.
