Profile Tab

WellSky will bring you to the Profile tab by default when you click on a client's name or search by their ID number. Some of the fields on this page can be found in other sections, like the Entry/Exit or Summary tabs. The Client Record and Client Photo should be updated from this tab.


Client Record - click the Pencil icon next to this section to change any of these fields

  • Name - must have first and last name filled out. If you are changing the client's name, please put the previous name in the Alias field
  • Name Data Quality - this should always be Full Name Reported since you have collected the first and last name
  • Alias - this field is optional. You can include client pronouns here, or alternate names the client may use
  • Social Security - you must ask clients for their SSN but they are not required to give it. If collected, this field should be completed. The last 4 digits are also acceptable
  • SSN Data Quality - this must be filled out for all clients and match what is in the Social Security field (ex: if you only collected the last 4 digits, select Approximate or partial SSN reported)
  • Sex - If you work at an agency that is funded by CoC, YHDP, ESG, RHY, PATH, VASH, SSVF or GPD funding you will need to fill this out for the client
  • U.S. Military Veteran - this must be filled out for everyone 18+


Client Photo - a client photo is not required. If one is added, it must be of the client's face. Any images that are not of the client should be removed by clicking Clear.



Client Demographics - click the Pencil icon next to this section to change any of these fields

  • Date of Birth - Every client needs a date of birth. If the client doesn't know it or prefers not to answer, use the date of January 1st (1/1/YY) and an estimate of their birth year. This should be updated on the Entry Assessment.
  • Date of Birth Type - Use Full DOB Reported for all birthdates, except if you had to estimate the birthdate use Partial or Approximate DOB. This should be updated on the Entry Assessment.
  • Race and Enthnicity
  • Gender
