Want to make one of your staff meetings fun this month? Have your HMIS users bring their laptops and complete The Reading a Client Profile puzzle game in the meeting together. 


Did you know?

You can learn a lot about a client from the Entry/Exit tab?


Being able to read a client's profile can help you make better decisions for the type of help a client may need. Using the Entry/Exit tab will show you the programs a client is currently enrolled in and what they have enrolled in in the past. You can even find other providers they have worked with to help coordinate efforts. 

 

Some examples of information that can be useful:

  • Which projects a person is currently enrolled in (projects with no exit dates) so you will know if they are eligible for certain projects. All project names list an acronym for what type of project it is. See the bottom picture for how we name our projects and what the acronyms mean.

  • Which projects a client has already tried (Would it be helpful for that client to visit that place again if they had a good experience? Would it be wise to use a different place if that one didn't work out?)

  • Any case managers or programs that you can contact to coordinate efforts with?
