When to Record a Follow Up

Follow Ups should be recorded for clients who exited any ES, TH, RRH, PSH, HP project into Permanent Housing. 

The Head of Household needs to have a follow up completed at 6 months and 12 months after program exit. This is only required for households who exited the project into one of the following permanent housing scenarios:

  • Owned by client, no housing subsidy
  • Owned by client, with housing subsidy
  • Permanent housing for formerly homeless (ie: Emerald, Camas, First Place Family PSH, Housing Plus, LIFT, Shelter+Care, Vet LIFT)
  • Rental by client, no housing subsidy
  • Rental by client, with VASH housing subsidy (must be veteran)
  • Rental by client, with GPD TIP subsidy (must be a veteran)
  • Rental by client, with other on-going housing subsidy  (i.e.: Section 8, SSVF, Rapid Rehousing, or other Rent Assistance Projects)
  • Staying or living in family, permanent tenure
  • Staying or living in friends, permanent tenure


If the household exited to any other exit destination, the 6 and 12 month follow-ups are not required.

How to Create a Follow Up Assessment (6-Month Review or Annual Follow up)

  • Contact the Household and collect the housing status for the 6 or 12 month exit anniversary date.
  • Go to the Head of Household's record in ClientPoint. Click on the Entry/Exit tab
  • Click on the Follow Ups icon (hint: you will only be able to create a Follow-up if the client has an Exit Date recorded

 

  • A pop-up menu will appear. Click on the Add Follow Up Review button

  • Select 6-month Review if this is 6 months after exit, or Annual Assessment if it is 12 months after exit. Even if it occurred a few days before or after the official anniversary, please record the Review Date as exactly 6 months or 12 months after the exit.


  • Answer the 3 questions in the assessment and then click Save & Exit


 

  • You will see the number of Follow Ups that have been completed on the Follow Ups icon

 



Follow Up Report


There is a helpful report in Business Objects that can be used both to see which Clients need 6 and 12 month follow-ups and the outcomes for those who have received follow  To access the report, please click on "Connect to Business Objects" in the right corner of your screen while logged into Wellsky.  

In a new tab, BusinessObjects will open to a folder named "lanecounty_live_folder."  Within that folder, you will navigate to a sub-folder, named "EntryExits - Move-In Dates - Interims - Follow-ups (post-exit)."

Note:  If you are new to Business Objects reports and your screen doesn't look like this, please use these instructions to update your settings: New ART Settings. 

Within this sub-folder, you will find the report titled, "Follow-ups (6 and 12 months) with Outcomes."  Below it, you will also see a PDF article with INSTRUCTIONS on how to utilize the report: 

To run the report, you will need to answer four prompts:  


  • EDA Provider: Always "-Default Provider-"
  • Provider: Your Project
  • Reporting Period Start Date
  • Reporting Period End Date (Plus One Day)


The report will then be available within your browser in Business Objects.  You can also use the download button at the top to view and edit the report as an Excel sheet: 


To read the report, please note the following sections:

  • At the top: You can see the project name and reporting period (orange)
  • Within the table, you can see: 
    • Client ID (yellow)
    • Exit Destination (green): Upon exit from the project, where did the Client go?
    • Entry / Exit Dates (blue): Date Client entered and exited the project
    • 6-Month Follow-Up & Outcome (pink)
    • 12-Month Follow-Up & Outcome (purple)
    • Follow-Ups Due (red)


Additionally, at the bottom of the report, you can also see the percentage of Clients housed at both their 6-month and 12-month follow-ups:
