Documenting a Client Death

Please follow these steps to document a client death:

Add DECEASED to the client’s last name

  • Click on the Client Profile tab and then click the pencil icon next to Client Record


  • Add DECEASED to the end of the last name and then click Save.

Exit the client from your project

  • The exit date should be the date of death. Reason for Leaving is Death and Destination is Deceased

Add an incident for the client with the date of death

  • Incidents are located on the Summary tab of the client profile. Click Add New Incident and put the date of death (approximate is okay) as the Start Date.

Please remember to email the client ID to the help desk so we can exit them from other projects and notify relevant providers.
