This assessment should only be completed by the VBNL case conferencing team.
In the client’s record in ClientPoint, click on the Assessment tab (last tab - if you do not see this, contact the Help Desk to get access)
The selected assessment will default to Veteran Status & Eligibility for Users with licenses at the Veterans Administration (Lane County) ADMIN and SVDP - Housing ADMIN levels. System Admins may need to select from the dropdown list.
Veteran Status & Eligibility Assessment
The purpose of this assessment is to prescreen for Veteran Status and program eligibility prior to Outreach and case conferencing. This assessment can be completed without client engagement using data from the systems used by the VA and SSVF staff.
Reporting - This data is not associated with an entry/exit. It can be reported in ART. This data is reported in the BNL and VBNLs.
