Once a Front Door Assessment (FDA) is complete and entered into ServicePoint, Lane County staff will review the FDA for data quality and completeness, and then determine if the household meets eligibility criteria for the CWL. WellSky will automatically put your FDA on a list for Lane County staff to review once all the following conditions are met:
- Client must have an entry into the Front Door project
- The Client’s “Relationship to Head of Household” must be “Self”
- In the assessment, the “Household Type” must be completed
- Does client have a disabling condition? question must be completed
Client must have an entry into the Front Door project
This entry is where you will fill out the client's Front Door Assessment. Once the client's FDA is accepted, they can be on the CWL for up to 6 months. This 6 months is calculated from the Project Start Date of your entry.
The Client’s “Relationship to Head of Household” must be “Self”
Only one person from a household should complete the Front Door Assessment. Typically, this is the most vulnerable adult in the household. This person must be marked as Self (head of household) on the entry.
Please do not include other household members on your entry! There are questions on the FDA that will collect the information we need to know about other household members.
In the assessment, the “Household Type” must be completed
The Household Type question is how we determine what list the client should be included on (Singles, Families, or Youth). This should be the household composition once the client is housed.
For example, if a client has no children living with them but is pregnant, they should be marked as Adult and Children HH since that is how they will be housed.
Does client have a disabling condition? question must be completed
This question is required to determine whether the client is Chronically Homeless. The answer can be Yes or No, but it must be filled out for the client's assessment to be reviewed.
