Creating a Front Door Assessment

Front Door tips:


Create the entry into your Front Door project like you would create a typical Entry/Exit. The instructions below give more information about the questions in the assessment.


Head of Household Information

  • The client should always be the Self (head of household) or they will not be sent to LC for review. Remember only the client you did the assessment with should be on this entry.
  • Remember to update the Front Door Agency and Assessor information. This lets Lane County know who to contact if there are questions about the assessment


Coordinated Entry Assessment table

An alternative housing opportunity has been identified, and household does not need to be placed on the CWL: Use Crisis Needs Assessment and Not Placed on the Prioritization list


No alternative housing opportunity has been identified, and the household does need to be placed on the CWL: Use Housing Needs Assessment and Placed on the Prioritization list

  • Use the Add button to access the sub-assessment
    1. Date of Assessment
    2. Assessment Location
    3. Assessment Type: Phone, Virtual, or In Person,
    4. Assessment Level: Crisis Needs Assessment or Housing Needs Assessment
    5. Prioritization Status: Placed/Not Placed on the Prioritization list
  • Comprehensive Assessor’s Name


Client Demographics

  • Make sure all these fields are completed


Client Contact Info

Click on the Add button and complete as many of the fields for contact information as possible. 

  • The contact info must be less than 6 months old and all old contact info should have an end date. Assessments must have provide a way to contact the client or it will not be accepted


Diversion Questions

Coordinated Entry Event table:

An alternative housing opportunity has been identified, and household does not need to be placed on the CWL: Use Yes

No alternative housing opportunity has been identified, and the household does need to be placed on the CWL: Use  No

  • Use the Add button to access the sub-assessment
    1. Start Date
    2. Date of Event: Same as Start Date
    3. Event: Problem Solving/Diversion/Rapid Rehousing intervention or service
    4. Problem Solving/Diversion… Event- Client housed/re-housed in a safe alternative: Yes or No


Participants may be exited from the Front Door Assessment (FDA) due to diversion without transitioning from houselessness to housing. An FDA exit indicates the participant has identified alternative housing and does not require placement on the Centralized Waiting List (CWL).


If the identified housing opportunity is no longer viable, the participant may return to the access point to explore other housing options or be added to the CWL. 


If the client was diverted, you will now need to Exit the Client:

  • The rest of the assessment does not need to be completed.
  • Scroll to the bottom of the page and Save the assessment.
  • Scroll back to the top of the assessment and click on the pencil icon next to the Exit Date to exit the client from the project. Enter the Exit Date, choose Left for Housing Opportunity, and choose the Exit Destination.

 

If the client was not diverted, continue filling out the rest of the FDA Assessment: 


Household Info

  • Answer these questions for how the client will be housed. For example, if client is a single adult but is pregnant, their household type would be Adult and Children and Household Size is 2.


Length of Time Homeless

  • These questions are based on client self report but also need to line up with documented entries in the client record. 
    • Ex: client says they have been homeless for the last 3 years but you see that they were housed in PSH 2 years ago. This needs to be documented in the 3 year history.
    • Ex: client says they have been homeless for the last 3 years but in their last FDA 8 months ago, they said they had stayed with friends for a few months. Use the most recent client self report
  • Prior Living Situation is where the client slept the night before your assessment. If this is not a homeless situation, they are not eligible for a FDA and their assessment will be denied.
  • Length of time homeless questions (Prior Living Situation, Number of times homeless, etc) must match what was reported in the 3 year history.


Current Living Situation

Current Living Situation - This only needs to be completed if you are assessing someone under the age of 25, AND they are currently doubled up or at risk of homelessness. Click Add to add the Current Living Situation, and make sure to answer all of the question listed below (they are also highlighted in the screenshot underneath)

  • Start Date / End Date / Information Date: All the same – day of assessment
  • Current Living Situation: Where they are currently staying
  • Is the client going to have to leave their current living situation within 14 days? If Yes, you'll also need to answer the next two questions:
    • Has a subsequent residence been identified?
    • Does individual or family have resources or support networks to obtain other permanent housing?

 


Host Homes

This only needs to be completed if you are assessing someone under the age of 25. 


Coordinated Entry Vulnerability Assessment

Each question needs to be answered with a Yes or No.


Disability Info

  • This is based on client self-report. The questions in the HUD verification table should match the other disability question in the table beneath it. 
  • See the HUD Verification Tables article for help with changing any No answers to Yes answers.


Intellectual Disability Section

  • Does anyone in the household experience an intellectual or developmental disability? This is the ONLY question that asks about the entire household. Therefore the answer in the HUD table does not need to be Yes for this answer to be Yes. The next two questions should be answered if the previous question is Yes.


Health Insurance Info

If the first question is Yes, the other two questions do not need to be answered.

If the first question is No, the following two questions must be answered.


Domestic Violence Info

This section can be left blank if the client is accompanied by another adult


Make sure to SAVE your assessment when it is complete.
