Referrals Report

TABLE OF CONTENTS


Use this canned report to see which clients have been referred to a project, or to see all referrals that have been sent to your agency. You can access the Referrals Report by clicking on Reports in the left menu. The report is called Referrals.




Report Prompts


Provider: To see referrals to a specific project, select your project as the Provider and leave the selection on the default "This provider ONLY". If you are an agency admin and you want to see referrals for your whole agency, select your ADMIN project as the provider and pick the selection "This provider AND its subordinates".

Referral Type: Select "Incoming referrals to provider"

Referral Status: Filling this out is optional. Outstanding will only show referrals where the outcome has not been updated yet. Closed will only show referrals where the outcome has been updated. Selecting ALL will show all referrals regardless of status.

Referral Outcome: This is optional. You will only get the option to fill this out if you selected Closed or ALL as the Referral Status.

Referral Date Range: It's recommended that you fill this out. Referrals will be included if they were created during this date range, not if they were updated.

Sort Order: This is optional. When you click Select, a pop-up menu will appear that allows you to pick columns to sort by, and which columns get priority in the sorting. We recommend Referral Date and Referral Outcome.


Once all columns have been filled out, click Build Report to load the report. After the report has run, you can click on Export Report to download it to Excel.


The Report Results will show each client and Referral. If you click on the name of the client, the system will take you to that client's profile. If you click on the Need Type, you will be taken directly to that client's referral. You can also hover over the Need Notes to see any notes.


Types of Referral Outcomes: 


  • AcceptedClient has been contacted and has indicated they want to be housed in this project.
  • Accepted on the Waitlist: (The Annex ONLY) Referral has been sent from the Family Shelter Cognito form and The Annex providers add these referrals to their internal waitlist.
  • Declined: The client was unable to be contacted, did not want to be enrolled in the project, was no longer eligible for the project, or the provider is declining the referral for some other eligibility-related reason.  (If choosing this option – please provide more information in the “If Canceled or Declined, Reason” section)
  • Cancelled: There was an error on the county end in making the referral or an error on the provider end in requesting the referral.  (If choosing this option – please provide more information in the “If Canceled or Declined, Reason” section)
