What form do I use?
The intake form you use is determined by project type and funding requirements. Form type is determined project-by-project, not agency. If you don't know what form you use, just look at the top of one of your client's assessments in either ClientPoint or ShelterPoint. The name may not match exactly but you should find LIGHT, BASIC, PLUS, or YHDP in the title.
