There are some instances where a client will transfer from one housing project to another of the same project type. Clients cannot be enrolled in two housing projects of the same project type at the same time. Instead of immediately enrolling the client in your project once they have been found eligible, you will need to wait to enroll them until they have exited from the first project.
When the client has moved to your project:
- Exit the client from the first project. Be sure to exit via the ShelterPoint Unit List if this is a ES, TH or PSH project. If this is not your project, then the original provider will need to exit the client. If you are not sure who that is, contact the Help Desk at HSDHelpDesk@lanecountyor.gov.
- The Exit Destination from the first project must match the Prior Living Situation on the new project.
- Enter the client into the second project. The entry date for the second project must match the exit date of the previous project.
Housing Move-in Date
If the client was housed at the time they transferred from the first project:
- Clear out the Housing Move-In Date (will have populated from previous enrollment) on your entry
- Create an Interim Review for the same date as your new entry and add the new Housing Move-in Date.
If the client was not housed when transferring from the first project:
- Make sure the Housing Move-In Date is empty on your new entry
- Create an Interim Review when client is housed and record Housing Move-In Date on that interim
Add service transactions for the new project using the same Provider as the new Entry/Exit.
