DQ for RHY Entry Assessments

This report checks project entries to ensure all required RHY assessment questions were answered. RHY projects are funded by Runaway and Homeless Youth (RHY), and a project is RHY-funded if there is [RHY-SOP] in the project name.

How to Run the Report

From WellSky's HMIS site, click on Connect to Business Objects.


Navigate to the folders:

   > lanecounty_live_folders

       >Data Quality

           >DQ for RHY Entry Assessments


To fill out the prompts:


EDA Provider: Do not use/leave as default provider

Project(s): Enter in your project name, multiple projects can be entered

Start Date: Enter in the start date of the date range you are interested in viewing

End Date (PLUS One Day): Enter the day after the end of the date range you need


Click on Run.

Navigating the Report



At the top of the report, you have some useful buttons.  The down arrow pointing to a line lets you download the report to Excel or similar spreadsheet program (orange box in image above) (recommended!).  Where it says "Overall - All Elements" in the purple box, this is a filter to choose to see all clients enrolled in your date range, only clients with complete assessments (All OK), and only clients with missing assessment questions (Missing Values ONLY).  


Reading the Report

This report evaluates if all project entry assessment questions were answered.  There are eight headers and a list of clients below. 

Clients with complete entry assessments (meaning they have every YHDP question answered in their project entry) will show up grayed out on the report.  



Clients with missing assessment questions will be in full color.  Missing fields will show up as "Null" with red text and highlighted red.  The green Ok fields do not need to be fixed.



The table below is set up to match the report sections (colored headers) and the abbreviations for  questions on the assessment (white text with black background).  Next to the abbreviations are the full text of the questions you find in the YHDP WellSky entry assessment.  Please use the following table to match the report columns to assessment questions in WellSky.

Entry & Exit Information

Other Tabs

These columns represent data errors that may occur for your project, but are not found within the project entry assessment.  These pieces of the data quality must be fixed in other tabs, which are listed in (parentheses) after the field name.  Refer to the Profile Tab helpdesk article for assistance with these first three fields.

This marks the beginning of the RHY entry assessment questions.  To find these / edit them, navigate to the client profile in WellSky, choose the Entry/Exit tab, and click on the pencil to edit the project start date.

HUD Verification Tables

There are three HUD Verification tables that you must complete.  Complete tables have a blue check mark at the top right, incomplete tables have a red triangle in the top right.  You must complete the table and answer the question after the table.  More help with the HUD tables can be found here: HUD Verification Tables Helpdesk article.
