Creating a Front Door Assessment

Front Door tips:


Create the entry into your Front Door project like you would create a typical Entry/Exit. The instructions below give more information about the questions in the assessment.


Head of Household Information

  • The client should always be the Self (head of household) or they will not be sent to LC for review. Remember only the client you did the assessment with should be on this entry.
  • Remember to update the Front Door Agency and Assessor information. This lets Lane County know who to contact if there are questions about the assessment


Client Demographics

  • Make sure all these fields are completed
  • The contact info must be less than 6 months old and all old contact info should have an end date. Assessments must have provide a way to contact the client or it will not be accepted


Household Info

  • Answer these questions for how the client will be housed. For example, if client is a single adult but is pregnant, their household type would be Adult and Children and Household Size is 2.


Length of Time Homeless

  • These questions are based on client self report but also need to line up with documented entries in the client record. 
    • Ex: client says they have been homeless for the last 3 years but you see that they were housed in PSH 2 years ago. This needs to be documented in the 3 year history.
    • Ex: client says they have been homeless for the last 3 years but in their last FDA 8 months ago, they said they had stayed with friends for a few months. Use the most recent client self report
  • Prior Living Situation is where the client slept the night before your assessment. If this is not a homeless situation, they are not eligible for a FDA and their assessment will be denied.
  • Length of time homeless questions (Prior Living Situation, Number of times homeless, etc) must match what was reported in the 3 year history.

Current Living Situation

Current Living Situation - This only needs to be completed if you are assessing someone under the age of 25, AND they are currently doubled up or at risk of homelessness. Click Add to add the Current Living Situation, and make sure to answer all of the question listed below (they are also highlighted in the screenshot underneath)

  • Start Date / End Date / Information Date: All the same – day of assessment
  • Current Living Situation: Where they are currently staying
  • Is the client going to have to leave their current living situation within 14 days? If Yes, you'll also need to answer the next two questions:
    • Has a subsequent residence been identified?
    • Does individual or family have resources or support networks to obtain other permanent housing?

 



Disability Info

  • This is based on client self-report. The questions in the HUD verification table should match the other disability questions in the table beneath it



Make sure to SAVE your assessment when it is complete.
